Initial New Patient Consultation

$1,250 one time

An in-person comprehensive history and physical at our Scottsdale office. The initial visit typically takes approximately 2–3 hours and is designed to establish the full clinical picture before treatment decisions are made.

  • Individualized laboratory and/or diagnostic testing when indicated

Breakthrough Membership

$120 per month
$1,395 annually

One straightforward membership option for ongoing care with Jason Keable and the Breakthrough Functional Medicine practice.

Ongoing visits are billed at $80 per 15 minutes.

Individualized Testing

Jason orders laboratory work and/or diagnostic testing based on your history and examination to gather more information or help confirm or rule out differential diagnoses.

Clinical Review

He reviews the testing and diagnostic data together with the comprehensive exam and develops a treatment plan using the best available evidence across multiple specialties.

Follow-Up & Ongoing Care

The included follow-up is used to review results and explain the treatment plan. Ongoing care then continues through membership at $120/month or $1,395 annually, with ongoing visits billed at $80 per 15 minutes.
